The film is set in British India in the year 1915. The story is about a young doctor, Govardhan Menon (Mohanlal), who is wrongly accused of bombing a train carrying 55 people including British officials. He is put in a cellular jail at Port Blair, Andaman and Nicobar Islands. Kaalapani shows the sufferings of hundreds of Indian prisoners in the cellular jail, including leading participants of the freedom movement. The extremely inhumane conditions faced by the prisoners in the jail are accurately depicted.

Kachra Seth runs an empire of collecting garbage, which is merely a front to cover-up for his other business - drug trafficking. He recruits a select group of people to carry out unpleasant tasks - such as beating someone up, or even killing someone. One of his recruits is Jaggu, who will do anything for a price. Jaggu loves Tina but is afraid to tell her. When Jaggu's mom is hospitalized, he comes to Kachra for monetary assistance, he is assigned to a task instead, and unable to complete the task he is apprehended by the police with the assistance of a librarian, and is subsequently found guilty and sent to prison. Jaggu's place is taken by Rajeshwar alias Raja, who is also attracted to Tina and even rescues her from one of Kachra's goons, Chabiley, and they both fall in love. When Jaggu returns from prison, he finds his mom has died, and his sister is missing, believed to have committed suicide. In anger, he concoctes a fool-proof plan to kill the librarian, and he does so. Jaggu does not know that the librarian is none other than Raja's dad, and Raja has sworn to hunt down his dad's killer, no matter what.

After completing his studies in London, Shakti Thakur (Anil Kapoor) returns to the small Indian town where his family resides. Accompanying him is his girlfriend, Anita (Pooja Batra), whom he is in love with and wants to marry, much to the disapproval of his family. After a few days, Shakti starts to feel that nothing much has changed in this part of India and longs to leave. He tells his father that he would like to sell his share in the family land and would open Restaurant business and settle with his girlfriend, to this his Father(Amrish Puri) urges him to stay in the village and uplift it with his education, by saying "A man gets Education not to become a selfish being but to uplift his uneducated brothers and society". Shakti does not agree with his father and decides to leave. He is unable to bear the animosity between the townspeople in general and in particular between his Zamindar dad, Raja Thakur (Amrish Puri), and rival Zamindar Birju (Govind Namdeo), none other than Raja's crippled brother.

Swami Amritananda (Mohanlal) arrives in Kerala on a mission to bring peace at the state, which is burning with communal riots. Akkineni Nagarjuna plays the role of a sincere Cop, Vamsi Krishna, who is sent undercover along with two other cops. Of which one cop is a close friend and associate to Vamsi played by Subhalekha Sudhakar, by Police Commissioner played by Giri Babu to track down a vicious and dangerous criminal named Raghuram played by Murali Mohan. Vamsi performs surveillance on Raghuram's daughter Geetha played by Amala, by getting into Geeta's opposite building's flat in a Residential complex, to gather details and whereabouts of her criminal father. Geeta lives with her aunt played by Sukumari. When Vamsi in guise of a telephone department inspector, pursues her and falls in love with Geetha, she reciprocates Vamsi's love interest initially but refuses him after she gets to know the truth that he's a Cop and is about to drag her father to prison. The rest of the story is about how Vamsi gets hold of Raghuram through Raghuram's daughter Geetha and what facts he learns about the crime operation of the real Criminal played by Sharat Saxena who deceives Geetha's father Raghuram who is actually an innocent victim, and in the process how the misunderstanding between Geetha and Vamsi gets cleared up, how Vamsi banishes the real criminal. The plot of the movie was inspired by the American movie Stakeout.
