The film celebrates the 10th anniversary of John Lloyd Cruz and Bea Alonzo's partnership as a love team and also their comeback film after their hit 2010 film entitled Miss You Like Crazy.
The film was well acclaimed and received mixed to positive reviews both from the critics and fans, and sets a box-office record in the Philippine Cinema. It grosses over PHP 260 Million, becoming the highest grossing non-MMFF film of 2012 and is currently the 9th highest grossing Filipino film of all time. It also won and received six several awards (including the cast and crew) in the 31st Luna Awards and received nominations in 61st FAMAS Awards, Golden Screen Awards, and Star Awards for Movies in 2013 respectively. Due to the film's success, it was screened internationally in selected countries.
Alonzo and Cruz also had their TV series entitled A Beautiful Affair that aired in ABS-CBN last October 29, 2013 and ended in January 18, 2013, is also part of their 10th-year anniversary as love team and the success of The Mistress.

Shirley Templo (Vilma Santos-Recto) is a ruthless woman. Many people deal with her strong attitude only for fear of her cold stare, or her outlash. She works as a librarian in a school, and lives in a compound that is owned by her ex-husband, Benito Salvacion (Tirso Cruz III). Many of the Salvacion family members that live near the compound, even including her eldest daughter, Dang (Dimples Romana), plead with her to sell it so they can make a profit, and she can move to a more suitable living area. Hard-headed as she is, Shirley refuses and feels betrayed by her two daughters, Dang and Cherry, for even siding with their father who left them nearly fifteen years ago. Even more upset, she finds out that Dang will want to move out of the Philippines to Australia. Leaving her alone in the Philippines since all her children moved out, Shirley decides to move to New York City with her youngest and only son, Mark Salvacion (Luis Manzano). Mark is unaware, however, that this supposed vacation of his mother is actually a permanent visit.

Agnes is a meek and dependent girl living with her long-time boyfriend Jerry. When she is invited to live with her estranged mother in California, Jerry pushes her to go through with the trip despite her protests, for him to focus on his work and build his career as an advertising executive. Agnes struggles to survive in the US and tries several times to return to the country, but Jerry insists on prolonging her stay as he struggles to save up for their marriage and support his parents and siblings. He encourages her to get a job and Agnes starts working as a caregiver. Agnes tries to convince Jerry to let her return again and reasons that she has the money to support herself but Jerry insists that as the man of the relationship, he should be capable of supporting her. Jerry starts to avoid Agnes, stressing Agnes out more. The long distance puts a strain on their relationship and ultimately ends when a scared and distraught Agnes is unable to contact Jerry after being put in danger one night. Time passes and Agnes becomes more independent and outgoing, working hard until she lands a job as a real estate agent. Meanwhile, Jerry travels to the US and visits Agnes, with the intention of getting her back. Jerry is overwhelmed by the cultural difference of the US and the Philippines, Agnes' success in her career, and the changes in Agnes herself. Despite this, they still manage to mend their relationship, but Jerry accepts that he is simply unsuited for the kind of lifestyle they have in the US. He leaves Agnes and decides to come home.
